Anomaly Event Descriptions

Introduction

The following are the anomaly event descriptions. A DICOM Study is the study (with images) sent to Synapse from a modality. The Destination Folder is the Synapse folder created by a RIS order or RIS Scheduler where the DICOM Study has been inserted.

Anomaly Events

Description

Additional Image Study not Reset

This occurs when Synapse receives additional images after the study status is updated to Dictated or Finalized. When this happens, the study status is NOT reset to Complete.

Invalid Time Zone

A study is sent to Synapse with an invalid time zone.

Example: The abbreviation or offset might be missing, the abbreviation is invalid, or the offset format is invalid.

Matched Existing Procedure using Description

Synapse received a study from DICOM with a Procedure Description matching an existing Procedure Code. This eliminates the generation of duplicate Modality_Procedure Description entries in the Synapse Procedure Mapping Table.

MG Study Completed wo CAD SR

When auto-completion is enabled, a mammography study auto-completes even though a CAD SR has not yet been received for that study.

Mismatch [CAD SR]

A mismatch was detected. This occurs if a duplicate CAD SR (same SOP instance EUID) was received for a different study than the original.

Mismatch [Image]

The Image Instance UID on the DICOM Study matches the one in Synapse, but the Accession Number is different than the Destination Folder.

Mismatch [Patient ID]

The Patient ID on the DICOM Study does not match the Patient ID of the Destination Folder typically provided by the HL7 feed.

Mismatch [Patient Name]

The Patient Name on the DICOM Study does not match the Patient Name of the Destination Folder typically provided by the HL7 feed.

Mismatch [Patient/Study]

Synapse receives a RIS update to an existing study, but the data in the RIS study-update specifies a different Patient ID than the one used in the existing study.

Mismatch [Series]

The Series Instance UID on the DICOM Study matches the one in Synapse, but the Accession Number is different than the Destination Folder.

Mismatch [Visit/Patient]

The Visit Number on the DICOM Study matches the Destination Folder but either the Patient Name or Patient ID is different.

Mismatch [Visit/Study]

The Accession Number on the DICOM Study matches the Destination Folder, but the Visit Number is different.

New Images Study Reset

Synapse resets the study status to Complete when a new image is received.

Order After Image Study Late

Synapse receives the HL7 order message after the image. This is common during RIS downtime and will typically be paired with a Study Reset anomaly message.

Radiation Dose Study/Patient Mismatch

Synapse looks at the Study Instance EUID, Patient Internal ID, and Accession Number. This anomaly occurs when there is a mismatch between these values and previously known values.

Study Without Order Study Absent

Synapse receives a DICOM Study and cannot find a matching Destination Folder. Synapse creates a destination folder and places the images there. It creates the anomaly so System Administrators know to send the order in and then merges and reallocates the images to this order.

Note: This anomaly was known as NewPatientStudy in previous versions.

Study Not Reset Per Sender Config

Occurs when Synapse receives additional images after the study status is updated to Dictated or Finalized. When this happens, the study status is not reset to Complete or Sent (depending on workflow).

This is set by Sender AE Title.

Zero Accession Number

A study is sent to Synapse with all zeros in the Accession Number field (a blank Accession Number will not cause an anomaly).

Zero Patient ID

A study is sent to Synapse with all zeros in the Patient ID field (a blank Patient ID will not cause an anomaly).
